With Form DS-11 you show your ID and hand in a photocopy of its front and back, on white 8.5 × 11 in paper, one side, not made smaller.1 The tool puts both sides on one Letter page at their real size.

The rules, word for word

What the State Department asks for.

The form, in short: "Present your original identification and submit a front and back photocopy with this form."2

Front and back on one sheet.

The rule is "on one side of the paper"; the apply-in-person page says "Use a single side of the paper".13

Neither asks for separate sheets. Unsure? Ask the acceptance facility.

Color

Color or black and white?

The current page doesn't say. A 2023 version asked for a copy that was "black and white, and single sided".4 Now it asks for white paper and one side.1

Grayscale matches the older wording.

Book or card, valid or expired, undamaged.1 Passport card.

Driver's license

In-state, fully valid, with photo.1 License copy.

Certificates and employee IDs

A Certificate of Naturalization or of Citizenship, or a city, county, state or federal government employee ID.1

A military ID? DS-11 accepts it,2 but copying one needs authorization.56 Ask first, or present another ID.

Your citizenship evidence needs a copy too.

"Submit your evidence of U.S. citizenship and a photocopy of the front." The back too if it has printed information.7 A birth certificate is a full page: use a copier.
